NDIS Psychosocial Capacity Reports

Psychosocial Assessments and Reports tell the NDIS about your psychosocial disability and how it affects your life and your wellbeing, so they can provide you with a funding plan to meet your needs.

What are Capacity Assessments and Reports?

A psychosocial capacity assessment or progress report from your providers gives the NDIS a detailed account of how your disability impacts your psychosocial capacity towards recovery and the life you want to live.

  • Best practice evidence-based assessments for NDIS psychosocial capacity are conducted. These are the LSP, HONOS and WHODAS, plus any suitable mental health-related assessments for you to discuss with your GP.

  • Please note that a capacity assessment is not a diagnosis, and you must engage a medical professional or clinical specialist for diagnosis if required.

  • The purpose of these reports is for NDIS planners to make funding decisions.

  • Reports are usually only required for NDIS plan review. We also provide short progress reports as requested by the NDIS.

What to expect

  • Our senior mental health practitioner will conduct a series of NDIS assessments with you. You’ll be asked questions about how your disability affects different areas of your life.

  • With your consent, your practitioner will collect evidence from other providers, your Carers and supports and write your report. The report will contain all the facts from your assessments and everything you tell us about your disability that you would like the NDIS to know.

  • Once your report is finished, you’ll have an opportunity to discuss your report to ensure you are satisfied with the description of your psychosocial disability impact.

  • You are provided an electronic version of your confidential report, so that you can send it to the NDIS, along with any other provider and/or medical reports, to be considered for the evidence-based needs in your funding budget.
